The Discharge Function Score of 2025 and What You Should Be Doing Now to Achieve the Best Outcomes

DESCRIPTION
Starting from January 2025, the Discharge Function score will replace the Total Normalized Composite scores in Value-Based Purchasing to measure functional outcomes. Join us to learn about how the Discharge Function score is calculated and ways to improve your functional outcomes, resulting in better OASIS outcome scores. Statistics show that patients with better functional outcomes will have increased opportunities to remain independent at home. Statistics also show that agencies struggle with responding to the GG items accurately. Learn how to measure functional outcomes accurately and initiate effective interventions to improve function.  

OBJECTIVES

1. Discuss the differences between the Discharge Function score and the Total Normal Composite score.
2. Identify OASIS items impacting the Discharge Function score.
3. Discuss strategies for 2024 to impact performance in 2025. 

Faculty: Lisa Selman-Holman, JD, BSN, RN, VP Education and Quality, McBee, part of Netsmart Lisa Selman-Holman, JD, BSN, RN, HCS-D, COS-C, is the VP of Education & Quality at McBee, bringing over 35 years of expertise in home health and hospice. As a registered nurse and home care attorney, she specializes in regulatory compliance. Lisa has held various roles, from field nurse to executive, managing accreditation surveys, acquisitions, and regulatory crises. In 2004, she founded Selman-Holman & Associates, offering consulting, coding and OASIS review services. Lisa is a renowned educator, known for turning challenges into achievements. Joining McBee in 2021, she leads education initiatives at the Post-Acute Academy, providing courses for home health and hospice staff and executives, including clinical, regulatory and onboarding requirements.

CONTINUING EDUCATION CREDITS
This program has been designed to meet the continuing education requirements for the Minnesota Board of Nursing for 1.5 contact hours. It is the responsibility of the participant to ensure that this program meets the licensing and continuing education requirements of their state board and to retain the required documents in their personal file. Attendees must participate in the entire presentation for contact hours to be awarded – partial credit will not be available.  To apply for nursing contact hours, attendance will be verified via zoom and a quiz must be completed by the attendee.  Certificates will then be issued by e-mail.

2024 Change: CEUs will NOT be available with the recording link. A second opportunity to view the program has been scheduled for April 17 from 10-11:30 a.m at no extra charge to the participant.
