The Benefits of Home Care

What is Home Health?
Home Health is care provided in the individual's home that typically consists of intermittent skilled care that may include nursing, physical therapy, occupational therapy, speech language pathology and social work. The goal of Home Health is to treat an illness or injury such that the patient is able to regain their independence as best as possible.

What is Hospice?
Hospice is a model of care that focuses on serving and comforting patients at the end of the their lives as well as supporting their loved ones. Hospice involves an interdisciplinary approach in the provision of medical care, pain management and emotional spiritual support. It can be provided in the patient's home as well as freestanding hospice facilities, hospitals, nursing homes and other long-term care facilities.

What is In-Home Care?
In-home care provides non-medical services using personal care givers who are trained to understand the nuances of long-term care. Personal care givers can help individuals with activities of daily living, such as meal preparation, medication reminders, laundry, light housekeeping, errands, shopping, transportation, and companionship.

Improved Patient Outcomes

  • Patients who have received care transition services through a collaborative partnership between a hospital and home care agency are significantly less likely to be readmitted to the hospital

Increased Patient Satisfaction

  • Patients using home care for both acute and chronic health conditions report higher satisfaction than individuals receiving care in in-patient settings
  • Rural Oregonians can more easily age in-place, increasing patient satisfaction and reducing stress
  • Professional in-home care provides companionship for individuals experiencing isolation and depression

Cost Effectiveness

  • Home care can reduce healthcare costs by managing disease and preventing hospitalizations
  • The average cost of care in a skilled nursing facility is $26,940 for a 60-day stay compared to $2,674 for a 60-day home health episode.
